She had to lie to save Ellie and gave them the responsibility to give her to someone who could raise her along with her knife. Anna is later shot by Marlene.

Joel and Ellie
In the current scene, Joel and Ellie are on their way to find the firefly members. They stop in the middle to find some supplies which they could use. Joel finds a guitar as they get closer to the Hospital but it is not in a really good condition. He also notices that Ellie is distracted but comes up with a plan to reach the hospital safely. They wanted to go on top of the hospital to have a better understanding of their surroundings.
Ellie reaches the top but gets distracted and drops the ladder while she passes it to Joel. However, he finds the staircase and makes his way to her. They have a heart-to-heart conversation later on. What Happened to Them?
Joel wakes up only to find that the fireflies have taken them, whom they were searching for. Later on, Marlene chats with Joel and tells him that Ellie is immune because she has had Cordyceps since birth and they will use it as a cure. However, he realizes that the removal procedure will cause Ellie’s death because it is in her brain. He wanted to meet Ellie, but Marlene did not allow her to meet him and explained the past to him. Therefore, she ordered the guards to keep him out of the hospital and kill him if he persisted. While going down, he kills the guards and takes their weapons, only to find Ellie. He kills several guards and the members during this incident.

Did Joel Save Ellie?
Joel takes down many guards and finally reaches the room where the surgery is about to happen. There were doctors, all set to perform the surgery on her. The doctor tries to fight Joel with a scalpel, but it is of no use since he shoots him directly. However, the assistants were spared and he asked them to remove all the machines from Ellie. He meets Marlene while taking Ellie down and she tries to convince him that Ellie would want to do the right thing instead of being killed in the wild. They get into an argument but Joel rescues themselves and gets into the car. Joel lies to Ellie about what happened and later shoots Marlene to escape.
Are Both of Them Safe?
Although their car breaks down along the road, the two manage to reach Wyoming. According to Joel, it will take them around five hours to walk back to Tommy’s. Ellie asks him to swear to her that all of the information he provided about what happened at the Fireflies hospital was true. She says “All right” and as the season ends, the scene cuts to darkness.
